This mandatory questionnaire is designed to help establish your general and medical profile. The results will play a decisive role in the management of your Health Passport.
Several factors, among them age and sex, can influence the interpretation of your clinical and physical results. Although some of these factors, like smoking, can be changed, others are permanent and unalterable (genetic predisposition, etc.). Each factor nevertheless plays a key role in how your results are presented.
By completing this questionnaire and updating it as needed, you will be making sure your Health Passport and the various Médicobox tools at your disposal are tailored to your individual profile.
Physical fitness and nutrition habits questionnaires
These questionnaires are optional, but the information they provide can be equally important in establishing your detailed health profile. They can be included in your assessment if you are following a fitness or nutrition regimen or if you simply wish to provide healthcare professionals with additional context concerning the overall state of your health.
The results from the fitness questionnaire will be used to draw up a general fitness profile and determine:

Similar to the fitness questionnaire, the nutrition habits questionnaire provides dietitians and other healthcare professionals with a plethora of information about your food choices and eating behaviours.
The detailed results will give your practitioners invaluable insight into everything from your culinary abilities to your dietary restrictions in order to:

Note that your test results will be automatically filtered by the platform to display only those that apply to you. This helps keep your Health Passport succinct, easy to peruse and directly relevant to you and your care.
The information you need
Although every test is unique, the layout of the results in the Médicobox platform is designed to be consistent from one page to the next so that it is easier for you to find what you are looking for.
Each page starts with a short definition of the selected risk factor to give you a better idea of what it is and what it means. It then shows your most recent test results, followed by a chart or table that puts this information in context, based on your status and the answers provided in the general and medical questionnaire.
Context and history
Not only does the Médicobox platform make your test results easier to read, it gives you the tools to track them over time. A progress chart will be included in your Health Passport as soon as a second set of results is entered into the system.
With this feature, you can see how your test results change during a given period of time. You can also use it to consult the individual results in your Health Passport by selecting the desired date in the corresponding drop-down menu.
Your physical fitness test results, which can be found in another section of the Health Passport, are presented in a similar way so you can track your performance and see how you rank compared with the normative values defined by healthcare professionals in this field.
Motivational programs and relevant studies
In an effort to put as many useful tools as possible at your fingertips so you can manage your health efficiently and effectively, the Médicobox Health Passport has been developed to comprise a number of programs and studies directly related to your test results.
The first of these programs is the Health Report Card, the aim of which is to raise your awareness about the importance of certain key factors that can have a short-, medium- and long-term effect on your wellness.
The Médicobox Health Report Card is not meant to be a predictive tool. It is simply a guide that points out certain areas where you can make changes in order to improve your health and even prolong your life expectancy.
Divided clearly into five sections, the Médicobox Health Report Card incorporates your questionnaire answers and your latest medical and physical test results and then generates your health and fitness scores. The result is an all-in-one overview of the current state of your health that lets you track your progress over time.
There are obviously some factors you cannot do anything about – your genetics, for example. But you can make changes to your lifestyle where appropriate. Nutrition, and exercise (physical and mental) are two things that can have positive repercussions on your health and wellness.
Similar to the rest of the Médicobox platform, the contents of the Health Report Card are based solely on the results of the tests in your Health Passport. The length of each report, and the notes in each section, will therefore vary depending on the number of tests involved.
You can also keep track of changes in your health status by comparing your current and previous scores in your Health Report Card. These will automatically be updated every time new results are added to your Health Passport or you make changes in your questionnaire answers to take new lifestyle habits into account.
The Health Report Card is an excellent source of motivation for achieving your personal goals. Not only does it provide you with a full rundown of your health status, it delivers your various test results in an easy-to-read, easy-to-interpret format so you can instantly zero in on any potential problems.
Metabolic syndrome is not a disease per se. It is a group of physiological and metabolic factors that significantly increase the risk of type 2 diabetes, heart disease and stroke among those who show three or more of the designated traits.
Because most of the risk factors involved are asymptomatic, metabolic syndrome is virtually impossible to identify without testing.
The five risk factors associated with metabolic syndrome are: obesity, hypertension, hyperglycemia, hypercholesterolemia and hypertriglyceridemia. Each of these can be considered to be a risk factor and a potential source of complications, but the presence of three or more signs indicates a case of metabolic syndrome, which makes an individual three times as likely to develop type 2 diabetes, heart disease or stroke.

Easily identify tests or vaccines due to be re-administered.
Sign up for email alerts, which will increase your odds of staying current.
Age, sex and several risk factors can have an impact on the need and frequency for select tests and vaccines.
For example, a 40-year-old woman with a family history of cancer should go in for a mammogram more frequently than a 20-year-old woman with no such background.
The reminder tool allows you to build your own lists of medical follow-ups and vaccines, and change the settings individually to reflect your needs.
Incorporating additional information into your health passport
Although the Médicobox interface and the tools it contains already accommodate over 40 of the most common tests, we know that some users have more specific needs that are not currently part of the platform.
Médicobox can easily be adapted to suit your needs. Simply upload scanned versions of the reports you would like to integrate into your Health Passport to share with your healthcare professionals, using a specially developed uploading interface.
These attachments will then become an integral part of your Health Passport. From that point onward, they will be directly consultable via the Médicobox portal and included in all print/email versions of the report.

Last but certainly not least, the Médicobox platform enables you to share individual test results, full or even customized reports with various healthcare professionals, family members or other recipients of your choosing, using the print and email icons at the top of every page.
